Instructions for obtaining CME
Credits

 THANK YOU for your interest in purchasing
Relationship-Based Care courses from PerforMax3. Below is
a link to the PerforMax3 store. You will be asked to
"Register" if this is your first visit. You will be able to
purchase, enroll, and then access the courses. There are two
steps before you can begin your training
immediately.
Step
1
Select the course/courses you wish to purchase, add them
to your cart and check out. You will need to create a store account.
Click Register and simply provide the requested information on the
registration page. At final checkout you will enter your Credit Card
information. Note: your credit card purchase will be processed
within 24 hrs. however, you will have access to your course(s)
immediately after you complete Step 2. http://store.performax3.com |
 Step 2
To begin training
now, you must register in the Civility Mutual Learning Management
System (LMS). This is where your courses and training records are
retained.
Click the "Configure Civility Mutual Account"
button, then, "Register as a New User." Enter the required
information and then click "Submit." You should receive a
message stating you have been successfully registered with a link to
the Civility Mutual LMS. Click the link and enter your username and
password to begin training.
